WHAT TO DO IF YOU HAVE BPH: WATCHING FOR SYMPTOMS
The patient who selects the watchful waiting option should continue regular visits to the doctor and be under observation if the problem should persist or get worse, the frequent checkups may save his life. The doctor will probably suggest lifestyle changes during the period of watchful waiting. The patient will be told to urinate regularly (or at least to try!). He will also be advised to avoid drugs or cold medicine that can worsen his condition.
The patient should closely observe the behavior of his prostate and alert the doctor if the condition worsens or if new symptoms develop. He should be on the alert for certain symptoms during the period of watchful waiting, as there is the possibility that the enlarged prostate could cause damage to the bladder or kidney. Also, malignant cells may develop during the waiting period and start to spread outside the gland to various parts of the body. If he detects even a hint of urinary retention, the patient should contact his doctor immediately. In the event of recurrent bladder infections, urinary retention, or other symptoms becoming intolerable, the patient may be a logical candidate for more aggressive treatment.
The patient should watch for the following symptoms:
Slow urine stream and dribbling
Frequent urination during day and night
Difficulty in starting or stopping urination
Painful and burning urination
Painful ejaculation
Blood observed in urine or semen
An aching pain in the penis, scrotum, testicles, anus, lower abdomen, or lower back
Swollen lymph nodes
If advanced cancer develops, additional symptoms could include fatigue, loss of energy, persistent swelling of one or both lower limbs and back, rib or hip pain, and frequent pain or stiffness in these areas. Sometimes a tumor in the spine can enlarge and squeeze the spinal cord, causing weakness or a numbing feeling in the legs. This is a very serious condition.
Acute Urinary Retention One symptom that requires immediate medical assistance is acute urinary retention. This condition could create a lot of damage to the bladder and kidneys. Under extreme circumstances, it can lead to death.
If complete urinary blockage develops and you cannot void any urine at all, you must see your doctor immediately. The doctor will insert a catheter (a hollow, flexible tube) through die urethra into the bladder in order to drain the urine; this is often referred to as a "ream job." It's a common procedure, but a most unpleasant one. It may only take a minute or two, but it will seem like a lifetime. Medical historians reveal that thousands of years ago, the Egyptians treated this problem by inserting reeds or copper or silver tubes through the penis and urethra to widen the urinary passageway and allow the urine to escape. This primitive and crude catheter probably saved many lives, but it most likely also caused the demise of numerous Egyptians.
If you go through this traumatic experience (as I did), you may start to think more seriously about terminating the watchful waiting period and having a surgical procedure. Yet the prospect of surgery might make you stop and reconsiderjust maybe you could wait a little longer.
